I confirm that my item(s) is/are lost/undelivered. I confirm that the good(s) ordered from ExpressgGolf.co.uk have not been received. I have checked with those within my household and neighbours and they confirm they have not received the goods. I understand that a false claim for compensation could result in criminal prosecution. I declare that, to the best of my knowledge, the information I have given on this form is correct and truthful. I also undertake to advise ExpressGolf.co.uk immediately if any lost items are subsequently traced and or delivered to refund ExpressGolf.co.uk any monies paid in compensation for these items. I agree to cooperate with ExpressGolf.co.uk, the delivery courier and/or the police in the event of any investigation into the missing goods.
